Membership Hold Request For COVID-19
During the COVID-19 pandemic, we understand that members may want to put their membership on hold temporarily.
You can place your membership on hold at no charge by submitting the form below and selecting a date to come off hold through August 31, 2021. If you need to put your membership on hold due to a medical reason, please submit this Medical Hold Form.
Please submit your membership hold request before the 15th of the month to stop the auto-draft process for the next month, which occurs on the 1st.
Please note that your monthly membership will automatically re-activate on the date you choose and your monthly fees will be billed on the 1st of each month.
Once your request has been processed you will receive an email to confirm your membership re-activation date. It is your responsibility to notify the Fanwood-Scotch Plains YMCA in writing 15 days prior to the re-activation date if you wish to further extend your hold or cancel your membership. You will not receive a reminder from the Y when your membership comes off hold. If you have any questions about your hold status you can log into your Daxko account or call the Welcome Center at 908-889-8880.
If you do not cancel or extend your hold status 15 days prior to your re-activation date, the YMCA will not issue refunds or credits for monthly membership fees that resume on or after that date. Please keep track of your re-activation date and contact the Y if you want to make a change 15 days prior to your date.

Please submit a separate form for each Youth, Teen or Young Adult membership.
