Medical Hold Agreement for the Fanwood-Scotch Plains YMCA
- A Membership may be put “on hold” for up to 3 months at a time with a doctor’s note stating that the member can not use the facility and the reason for absence.
- The doctor’s note must be sent to the Membership Engagement Director prior to putting the membership “on hold.” Please email a copy to Kristine Keating at kkeating@fspymca.org or mail to 1340 Martine Ave., Scotch Plains, NJ 07076.
- After 3 months, the membership will be active again and the member’s monthly draft payments will resume automatically.
- If the member still needs another 3 months, they must produce an additional doctor’s note and the membership will be put “on hold” for an additional 3 months. After that, the membership will become active again and the member’s monthly draft payments will resume automatically.
- A membership may be put “on hold” a total of 6 months in a calendar year.
- If a member needs more than 6 months to recover, the membership would then be terminated and it would be the responsibility of the member to sign up with a new membership when they are able to resume normal activities. The Joiner’s fee would be waived if you rejoin within one year. After one year, a Joiner’s fee will be charged.
- It is the responsibility of the member to fill out a cancellation form when they are terminating their membership due to illness.
Please note: This agreement will apply to all members on your account. If you would like some family members to remain active, please complete a membership change form to change their membership type.
