Membership Termination Request

Please submit your termination request before the 15th of the month to stop payment for the next month.

Please complete the form below. If you have a Family or Senior Couple membership, you do not need to submit a separate form for each member. Please submit a separate form for each Youth, Teen or Young Adult membership. You will receive an email to confirm your cancellation. Please retain this for your records. If you do not receive an email confirmation within 7 days, please contact membership@fspymca.org.

If you are currently enrolled in a program, your membership must remain active until the program is complete.

If you are cancelling your membership and have a credit on your account, you can use your credit as follows when you re-join the Y:

  • All system credits issued prior to July 1, 2021 will expire June 30, 2022.
  • All system credits issued July 1, 2021 and after will expire within 1 year of the issue date.
  • Program credits can only be used toward other programs.

The Fanwood-Scotch Plains YMCA will not issue a refund for unused system credits.
