Membership Termination Request

Please submit your termination request before the 15th of the month to stop payment for the next month.

Please complete the form below. If you have a Family or Senior Couple membership, you do not need to submit a separate form for each member. Please submit a separate form for each Youth, Teen or Young Adult membership. You will receive an email to confirm your cancellation. Please retain this for your records. If you do not receive an email confirmation within 10 days, please contact membership@fspymca.org.

If you are currently enrolled in a program, your membership will remain active until the program is complete or cancelled. Cancelling your membership does not cancel your program registration. You must complete a separate change form for the program you wish to cancel before your membership can be terminated.
