Membership Termination Request
In response to the current COVID-19 closure, we understand that members may want to cancel their membership. During the COVID-19 closure we are waiving the 30-day required notice, but you will need to submit your request before the 30th of the month to stop payment for the next month, which occurs on the 15th. For example, we will need to receive your request on April 30 to stop the May payment.
Please complete the form below. If you have a Family or Senior Couple membership, you do not need to submit a separate form for each member. Please submit a separate form for each Youth, Teen or Young Adult membership.
If your child is enrolled in Child Care, School Age Child Care, or Summer Camp and you would like to keep their spot in the program, please choose the membership hold option.
